Please review this Tennessen Warning (privacy notice) before providing information or data to Washington County.

What is the purpose and intended use of collecting data? 

The Self-Help Clinic is a voluntary program provided by the Washington County Law Library. You should only provide information you are comfortable sharing. 

The only information you are required to provide to begin as a program participant is your name, phone number, the other party’s name, whether you have legal representation, and a description of your legal issue. The court file number is optional. However, providing the court file number, if you have one, allows the volunteer attorney to look up your case on Minnesota Court Records Online.

We ask that you self-certify that your income is below 350 percent of the Federal Poverty Guidelines because that is required for participating volunteer attorneys to receive Continuing Legal Education credit for their pro bono services. Information may also be used for internal tracking, reporting, and program improvement purposes.

Is the data legally required, may I refuse? 

You are not legally required to supply any information to Washington County.

Are there any known consequences arising from supplying or refusing to supply private or confidential data? 

If you refuse to provide your name, phone number, the other party’s name, whether you have legal representation, a description of the legal issue, or sign the disclosure statement, then you will not be able to participate in the services provided.

If you are a minor, you have the right to request that private data about you be kept from your parents. You must make this request in writing and explain why you wish this data to be withheld and what you expect the consequences of sharing the data with your parents would be. If Washington County agrees that withholding the information from your parents is in your best interests, the data will not be shown to your parents as long as withholding data is consistent with state and federal law.

The identity of other persons or entities authorized by state or federal law to receive the data.

Information may be available to designated County employees to provide you with customer service and for administrative purposes. We will also provide the data to the volunteer attorney providing clinic services. Others that may access data include the Minnesota State Auditor, persons or entities with your written consent, persons authorized under a court order, and other entities and persons as allowed under state or federal law.
